| Resumen: | Se revisa la historia de la psiquiatría comunitaria, cuyo curso puede rastrearse seguramente hasta los albores de la historia. Siguiendo a Foucault, la aparición de los asilos y hospitales mentales habría coartado la continuidad de dicho afronte, cuya reintroducción aparece ahora como una "cuarta revolución psiquiátrica". Se incide luego en los factores que, a nivel mundial, han influido en el auge reciente de la psiquiatría comunitaria. Se describe con detalle la avanzada organización actual al respecto en el Japón. Se concluye en la necesidad de garantizar la excelencia de la atención comunitaria, poniéndola a salvaguarda de ciertas críticas emergentes. (AU)^iesThe history of Community Psychiatry is reviewed, whose origin can probably be searched back to the beginning of mankind. Following Foucalt's concept that the Asylum and Mental Hospital growth as institutions would have cut off the continuity of that model whose reintroduction appears now as the "Fourth Psychiatric Revolution", the autor points out the world factors that have influenced the new apogee of Community Psychiatry. In this respect the Japanese present advanced organization is described. The autor concludes that the excellence of psychiatric community service needs to be gurantee, protecting it from some emergent criticism. (AU)^ien.
